Data Summary

  • Assistant Professor Data Summary cover page
    • On the top portion of the cover page, please notate the name, primary department’s name and percentage, and review number. There is no “Split Appointment” check box. Therefore, draw a line through “Joint” and type next to it “Split” and notate the secondary department’s name and percentage.
    • If the candidate is maintaining a without salary affiliation with the current department, check the “Joint Appointment” box and indicate the department
    • The Present Status and Proposed Status sections need to be completed in their entirety
  • History Record
  • Joint Appointment waiver form (if applicable)

Supplemental Documents

  • Self-statement (strongly encouraged) – should indicate the candidate’s reason for seeking a split appointment and describe their contributions to diversity.

    • Separate votes are required for cases involving Joint Appointment, Joint Appointment waiver, or both
